On July 6th, 2016 UCLA Health implemented a new event reporting system called Safety Opportunities for Improvement (SOFI) for reporting of near miss and actual events associated with patients, employees and visitors.
At UCLA Health, we constantly strive to improve both patient safety and outcomes. The best way to do this is to build a strong culture of safety by enabling individuals to communicate actual or near miss events and opportunities for improvement. In turn, key stakeholders are made aware of situations and action plans are developed to improve care. The event reporting system is the primary source in UCLA Health that helps us identify opportunities for improvement.
