  1. What is my login and password for SOFI?

    • Please use your AD login to access SOFI.
  2. Once I’ve started a SOFI submission can I save it to complete it later?

    • Yes, events submitted into SOFI can be saved and completed at a later time.
  3. What is the timeframe in which reports regarding adverse events or near misses should be submitted into SOFI?

    • SOFI reports need to be submitted within 24 hours of the occurrence.
  4. Why should I submit an adverse event or near miss into SOFI?

    • By submitting an adverse event or near miss in SOFI, you are providing key information about variances in patient care that are often associated with care processes and systems. We can learn from those variances and improve processes and systems to prevent future similar occurrences.
  5. What happens after I submit an event into SOFI?

    • After an event is submitted, an email notification is sent from the system to department and category managers for review. The SOFI report will also be reviewed by Quality Management. Follow-up action(s) will be taken by specific departments as appropriate. Click here for current workflow
  6. Do I mention that I filed an incident report in the patient's record?
    • No. SOFI is an internal reporting system and should not be indicated on the patient's record. Also, all information on SOFI is protected and confidential, therefore you should not copy any notes from the patient's chart onto the SOFI report.
  7.  Can I view a SOFI report after it is submitted?
    • No, reporters cannot access completed events. For questions regarding action taken on events submitted, please contact your department leader. For any other questions or concerns please email sofi@mednet.ucla.edu
